Targeting tuberculosis 'hotspots' could have widespread benefit

Reducing tuberculosis transmission in geographic "hotspots" where infections are highest could significantly reduce TB transmission on a broader scale, according to a study led by researchers at the Johns Hopkins Bloomberg School of Public Health. An analysis of data from Rio de Janeiro showed that a reduction in TB infections within three high-transmission hotspots could reduce citywide transmission by 9.8 percent over 5 years, and as much as 29 percent over 50 years. The study was published May 28 by the journal PNAS.

"Targeting treatment of 'core groups' as a way to reduce community-wide transmission is common with diseases like HIV and malaria, but is less accepted as a mantra for TB control," said David Dowdy, MD, PhD, ScM, lead author of the study and assistant professor in the Bloomberg School's Department of Epidemiology. "Our findings suggest that hotspots containing 6 percent of a city's population can be responsible for 35 percent or more of its ongoing TB transmission. Controlling TB in these hotspots may have a similar impact on long-term, community-wide TB incidence as achieving the same targets in the remaining 94 percent of the population."

For the study, Dowdy and his colleagues developed mathematical models for TB transmission using surveillance data from Rio de Janeiro. Each model tested different scenarios for TB transmission between the hotspot and the rest of the community. Co-infection with HIV was also factored into the model.

According to the study, reducing TB transmission rates in the hotspot to those in the general community reduced citywide TB incidence by a mean 2 percent per year over the first 5 years. By year 50, TB incidence was reduced by 29.7 percent, reflecting a 62.8 percent reduction in incidence in the hotspot and a 23.1 percent reduction in the remaining community.

Tuberculosis infects more than 8.8 million people worldwide, resulting in 1.4 million deaths each year. The disease is known to cluster in hotspots typically characterized by crowding, poverty and other illnesses such as HIV. Nevertheless, TB transmission appears to be more homogeneous than that of many other infectious diseases, in which 20 percent of the population may generate 80 percent of infections.

According to Dowdy, "TB may not follow the same '80/20' rule that we see in parasitic or sexually transmitted diseases, but the '35/6' rule seen in our study suggests that targeting hotspots is still the best way to control TB in a community."

38% of Eurovision searches happened on mobile devices during the grand final

Did you enjoy the warm weekend across Europe with a nice BBQ and some relaxation in front of the TV with the Eurovision Song Contest? If you were watching the exceptional vocal achievements of the contestants like 100 million Europeans did last Saturday, it is likely that you had a smartphone or a tablet device in your hand at the same time.

From our recent Our Mobile Planet research, we know that consumers actively use their smartphones when multi-tasking with other media and more than 50% of Europeans do this while watching the TV. We therefore wanted to understand how Eurovision viewers were using their mobile devices during the finals by analyzing Google search results.

Top 10 mobile Eurovision countries - share of Google search queries related to Eurovision

Looking at related search queries from all 42 Eurovision countries on the day of and day before the finals of the Song Contest, we saw an increase in the share of smartphone and tablet searches by nearly 50% from Eurovision 2011. Consumers in a handful of European countries were particularly savvy, actively using their mobile devices to learn more about  the mind-blowing performances they saw on screen. More than 54% of Eurovision fans in Denmark searched from mobile devices, along with more than 40% in both Switzerland and Norway. Additionally, Denmark and Norway had the highest share of Eurovision searches conducted on tablets with 11% of Norway’s and 14% of Denmark’s Eurovision queries occurring on tablets.

Looking at a timeline of searches for Eurovision (normalized based on total search queries for each device type), show us that these mobile and tablet searches were largely complementary to desktop searches. At the primetime of the Eurovision finals, on the evening of May 26th,  we observed that people were switching to their smartphones and tablets with more than 38% of queries coming from mobile devices. This is similar to previous search data showing that tablets come out as the sun goes down. The large screen and compact form factor of tablets favour usage in front of the TV and from previous studies we know that the top browsing location is on the couch.

So, how can advertisers take advantage of all these multi-tasking mobile consumers? Make sure that you are on their screen with Google Mobile Ads so that you can get in front of them at the right time with the right ad formats. This way, if a customer sees your TV commercial and then picks up their phone to find more information, you may show up at the top of the results.

You can easily show your ads only when your customers are on their couch looking for information thanks to custom scheduling in AdWords. Or, you can configure your account to bid more aggressively at the times of the day that correspond with your television advertisements.

And don’t forget to add a mobile specific call-to-action such as a clickable phone number for potential customers holding a phone in their hand. You should also ensure that the site your users see on their smartphones is mobile optimized for the best user experience. While most Russian aviation experts contacted today dismissed the sabotage theory, they say there is a deepening mystery about how Russia's most modern civil aircraft, with all its systems apparently functioning perfectly, came to slam into the side of a mile-high volcano during a routine demonstration flight.

"All the theories put forward so far are badly flawed, there is a shortage of hard information and there are a lot of irresponsible rumors," says Roman Gusarov, editor of Avia.ru, an online aviation journal. "I am afraid that Russia is not going to emerge from this story without taking a black eye."

Citing an unnamed GRU general, Komsomolskaya Pravda?claimed that electronic jamming of the plane's on board equipment is the most plausible explanation for how the jet, which was making a demonstration flight out of Jakarta airport with 45 people aboard, smashed into a mountainside even though an initial investigation has found that its terrain and collision avoidance systems were all functioning properly.

"We are investigating the theory that it was industrial sabotage," the GRU officer is quoted as saying. He said that Russian intelligence has long monitored the activities of US military electronic specialists at the Jakarta airport.

"We know that they have special equipment that can cut communications between an aircraft and the ground or interfere with the parameters on board," he said. "For example, the plane is flying at one altitude, but after interference from the ground onboard equipment shows another."

The investigation has so far turned up the plane's "black box" cockpit voice recorder, which shows that no system-failure alarms went off during the plane's final minutes, nor did the crew take any audible emergency action. But the aircraft's digital flight recorder, which monitors flight systems and engine performance, remains missing in the rough jungle terrain around Mount Salak, where at least seven other deadly air disasters have occurred.

Big questions

The biggest question about the crash is why the SuperJet's pilot,?Alexander Yablontsev, one of Russia's most experienced test pilots, requested permission to descend amid a rainstorm in a notoriously mountainous area ? and why a ground controller in Jakarta granted that permission.

"Maybe he didn't see that the plane was heading straight at the mountain. On the other hand, we don't rule out the possibility that this was deliberate industrial sabotage to drive our aircraft from the market," an unnamed official at Sukhoi, the plane's manufacturer, told Komsomolskaya Pravda. "Fortunately, we don't foresee any loss of orders [for the SuperJet]."

Azerbaijanis form the largest minority population group within Iran ? some 24 per cent of the total. Recent dissent within Iran among the Azeri population has underscored the potential for creating internal political problems for the government in Tehran.

Bankia seeks US$24-billion bailout as debt worries mount

The Bankia group, a Spanish lender nationalized earlier this month, will seek 19-billion euros (US$23.8-billion) of government funds as it provisions against real estate and non-property loans.

The group will ask the state?s bank rescue fund to provide the money by buying shares in its parent company, Banco Financiero y de Ahorros, it said in a filing to regulators yesterday after a meeting of the board of Bankia SA, its listed arm. Bankia was among three Spanish banks that had their credit ratings cut to junk by Standard & Poor?s yesterday, citing Spain?s weakening economy. S&P lowered Bankia to BB+ from BBB- and BFA to B+ from BB-, the ratings company said in a statement. Banco Popular Espanol SA and Bankinter SA were reduced to BB+ and Banca Civica SA was cut to BB.

Bankia announced yesterday a restated 2011 loss of 2.98-billion euros, ?without reservations from auditors.? The company plans to reduce its board to 10 members from 18, with only Chairman Jose Ignacio Goirigolzarri and Chief Executive Officer Francisco Verdu remaining, it said in a filing.

Goirigolzarri will brief reporters on the restructuring plan today at 10 a.m. in Madrid and the bank will hold a webcast for analysts and investors at 12.30 p.m.

Spain said May 9 it would nationalize Bankia?s parent, making the state the biggest shareholder in the listed unit, just days after Chairman Rodrigo Rato resigned and the lender released 2011 accounts that weren?t signed by its auditor.

More Provisions

De Guindos on May 11 ordered lenders to set aside 30 billion euros to cover still-performing real estate loans, adding to demands made in February for provisions and capital of 53.8-billion euros.

Spain?s 10-year bond yield has climbed to 6.31% from 5.73% on May 7, the day Rato announced his resignation. Bankia has declined 58% in Madrid trading, to 1.57 euros, since selling shares to the public in July. The stock was suspended on May 25.

The additional 4-billion euros that the bank is seeking to cover real estate takes its total provisions for those assets to 12.7-billion euros. In addition to 5.5-billion euros to cover non-real estate lending under an ?adverse? stress scenario, the group needs 6.7-billion euros to clean up stakes in companies and tax assets, the group said.

?Big Number?

The group expects the state rescue fund, known as the FROB, to take control of BFA through the capital increase in June. After that, the Bankia unit will be recapitalized by a 12-billion-euro rights issue guaranteed by BFA that will probably go ahead in October, the group said.

The Bankia group will have a so-called principal capital ratio, a measure of financial strength, of 9.8% and a ratio of provisions to real estate assets of 48.9% after the cleanup, it said.

De Guindos told parliament on May 23 that Spain would provide as much public money as necessary for the Bankia group as he said it was a ?specific case,? without any read-across for the rest of the industry.

?It?s a big number for the restructuring, much more than the government had previously required, and shows the importance of going in and having a good look at the assets,? said Daragh Quinn, an analyst at Nomura International in Madrid.

Si bien es una funcionalidad los desarrolladores agradeceremos mucho y que facilitar? al usuario el consumo de servicios y bienes digitales, no deja de ser un coleteo de pez fuera del agua?por parte de Google para intentar equiparar su Play al AppStore de Apple, que ya implementa Apple Suscriptions desde hace m?s de un a?o, e intentar subir la rentabilidad de las aplicaciones para Android en general, muy bajas debido a la poca cultura de compra que existe entre los usuarios de la plataforma si los comparamos con los de iOS o Windows Phone.
